Welcome to PelviCore Physical Therapy!  Whether you inquired about pelvic rehab yourself or your physician reached out to me to contact you, I personally want to greet you and let you know that I'm sorry that you’re having these issues, but you've come to the right place for your rehab needs! 

Below is some info and answers to FAQ’s most new patients have that I’d like to share prior to scheduling appointments.

Insurance/Payment:  
I don’t know if you had a chance to look through my website, but like to inform prospective patients up-front that I am out-of-network for both for in-person and telehealth care, so I do not take insurance.  Most of us certified specialists do not accept insurance so we can dedicate the necessary time and care to help our patients get to recovery without restrictions and limitations that inevitability comes with being insurance provider. My patients self-pay at time of service and I provide them a detailed receipt with all the insurance codes for them to submit to their insurance companies. The insurance companies then reimburse them directly at whatever rate your particular policy reimburses.

Even though I don't take insurance and this may not apply to you, I still need to let you know that I cannot treat you if you have Medicare part B  as your primary insurance— part C with out-of-network benefits through a commercial insurance is ok.  If you do have Medicare part B, please let me know and I can try to refer you to a colleague. PT's can only treat Medicare part B and HMO patients if they are in-network providers of Medicare, which I am not.

Safety 
Due to ongoing COVID-19 and other contagious viruses, I am seeing patients both in-person visits and/or virtually through telehealth appointments for those who cannot or do not want to come in person. For in-person appointments, I now have a "Conditional Mask Optional" policy in the clinic, which means mask optional unless you have any symptoms that may be contagious, you’ve tested positive, or if you’ve been exposed.  In those cases, I would decide to either ask you to mask or cancel if your symptoms are bad enough.  Not only do I have an immune system problem, but I have several pregnant and postpartum patients with newborns at home.  It’s my goal to keep patients, me, and all of our families safe and also so the clinic can stay open because if I get sick, then no patients can be seen. 😁 Masking guidelines may change depending on community spread.

Fees:  My fees are time-based and are as follows:

  • • New patient Initial Evaluation (75 min)— $320 (telehealth and in-person)
  • • Follow-up visit (50 min)— $220 (telehealth and in-person)
  • • 30-min Follow-up visit— telehealth only— $110
  • • 15-min Follow-up visit— telehealth only— $55

PT Prescription 
In case you don't know, VA law states that physical therapy requires a prescription from a physician (or nurse practitioner, PA, chiropractor, etc.) and it must dated within the last 6 months to be valid. If you have one— great. Hopefully it’s dated within the last 6 months, so it’s still valid. If not, we may or may not need to secure one for you before your first visit depending on if you will be seen in-person or via telehealth.  Virginia law requires PT scripts for any invasive procedures— pelvic floor work qualifies as invasive and most patients require that as part of their therapy treatment plan.   There is little more wiggle room with telehealth appointments since they are not invasive, but they are limited to 30 days without a script.  Any need for PT beyond 30 calendar days will require a PT script, but we’ll have plenty of time to secure one.  If you need a script and don’t have one, but have already discussed this problem with a physician (or nurse, PA, midwife, or chiropractor) recently I can assist you by faxing a script request to the provider and hope that he/she will agree to sign it. You would need to provide the provider's name, phone, and fax number, which is part of the Registration Form (see attached).

Scheduling:  
Although I’m accepting new patients, my schedule is always very full, so scheduling can be a bit challenging. I try my best to get new patients scheduled as soon as possible, especially those who are pregnant or having acute pain.  I keep a very active wait list to be able to get patients in sooner as the schedule constantly changes when patients cancel or are discharged.  Patients are usually seen once per week, depending on the problem and my schedule.  I also can’t tell you how often you would need to come or how many total visits you might need until after a few visits to see how your body responds to treatment. Regular patient care hours are: Tue-Thu 8:00am-4:00pm (last patient at 3:00pm) and some Fridays 8:00am-12:00pm.

Cancellation Policy: 
I require at least 48 hrs notice when canceling to give me the opportunity to fill your slot.  There is always an very active wait list and canceling too late doesn't give others the opportunity to come n for much needed therapy.

Initial Appointment:  
Since the initial appointment is primarily a lot of talking, history taking, education, etc, especially for those patients with chronic symptoms and long medical histories, I do many of those appointments via Telehealth (restricted to those who are physically in VA during the visit) and then start in-person appointments the second visit, when I do both a physical exam in conjunction with treatment. There are some cases that in-person initial appointments are preferable, like prenatal patients and those with very simple and brief medical histories. Let me know if you prefer to be seen in person the first day because some openings for new patients can accommodate telehealth only.  Since the pandemic, almost all insurance companies are now reimbursing telehealth visits as if they were in-person visits, but I highly encourage you to contact your insurance company to confirm they will reimburse your telehealth visits and to ask if they require specific codes to ensure your claims will get processed properly. I have attached an Insurance Benefits Worksheet with details on the types of questions you can ask of your insurance company so you know up front what they are willing to cover for both in-person and telehealth therapy and you won’t be surprised after claims are processed. Note: Due to state licensure laws, I am able to do telehealth with all patients who are physically located in Virginia. If you reside outside of Virginia, please let me know and we will work around that.

After you schedule your first appointment, you will receive both admin/consent forms to sign as well as medical history forms to complete.  All forms must be returned to me at least a day before to your first visit for me to be familiar with your case and maximize my time with you.

Also, for full disclosure to help you decide if you want to move forward with me for your PT needs, I want to share up front that I bring my puppy, Maggie 🐶, to my office.  She is an adorable, (extremely) friendly, hypoallergenic, non-shedding 20 lb. mini Goldendoodle, who is leashed in a corner with a crate. She has become a great addition to my office, providing emotional therapy while I do the physical therapy 😊, but I want to make sure you’re comfortable with that.
